HCF Health Coaching for Knee Osteoarthritis provides eligible members* with personalised support from experienced health coaches to help improve knee osteoarthritis symptoms, mobility and quality of life, and connect members to evidence-based health programs.

HCF Health Coaching for Lower Back Pain

HCF Health Coaching for Lower Back Pain provides eligible members* living with persistent lower back pain (more than 3 months) with personalised support to help manage pain and improve movement. 


What you’ll get

  • Personalised assessment and improvement plan for chronic lower back pain.
  • Practical, evidence-based strategies to help manage pain, improve movement and quality of life.
  • Guidance on physical activity, exercise and lifestyle factors that may be playing a role in your back pain, how chronic pain can affect your emotional wellbeing and what you can do about it.
  • Ongoing coaching support delivered from the comfort of your home.

What you’ll get

  • Personalised assessment of your fall risk and health needs.
  • Tailored fall risk reduction plan with practical strategies to help improve strength, balance and stability.
  • Guidance to help create a safer home environment and maintain or increase independence. 
  • Ongoing coaching support delivered online or over the phone. 


Expected outcomes

  • Improved strength, balance and physical stability.
  • Reduced risk of falls and greater confidence in movement.
  • Increased independence in day-to-day activities and at home.
  • Improved overall wellbeing and quality of life. 


Eligibility

  • Must have held HCF hospital cover for 12 months (excluding Accident Only Basic and Overseas Visitors Health Cover) and be up to date with premium payments. 
  • Must be aged over 65 years old (or 50 years old or older for Aboriginal and Torres Strait Islander members), or younger with a personal concern about falling.
  • Meet clinical eligibility criteria.

You’ll also have the option to speak with your health coach before your plan is created. Your coach can provide ongoing guidance, check in regularly, and help you track your progress to support your health and wellbeing over time.

To make sure you’re on the right track, each member will be invited to complete a feedback survey 2 weeks after completing a coaching session.
