Notice of transfer of HCF Eyecare business records to HCF
The Hospitals Contribution Fund of Australia Ltd (HCF) acquired the HCF Eyecare business from Eyecare Holdings Pty. Limited effective 1 July 2019.
The business records for the HCF Eyecare business (which includes all current and former customer records (including personal information) have been collected by HCF. The business records of each customer will be not be used or accessed by HCF unless the customer provides their consent on their next contact with the HCF Eyecare business. Should consent not be given, HCF will be unable to access the customer’s business record including their patient records and HCF will need to open a new record if the customer wishes to remain a customer.
Please be aware that HCF may still use the customer’s personal information that it holds prior to the collection of the HCF Eyecare business records (which may include their personal information in those records).
