HCF :: The HCF Healthy Heart Program ::

HCF would like to assist you to manage your health and maintain a healthy lifestyle.

HCF has been looking into initiatives to assist you with your heart condition by improving self-management of your illness and addressing lifestyle behaviour which may impact on your health.

The HCF Healthy Heart Program is a 12 month telephone based program for members with congestive heart failure or coronary artery disease, at no charge. It is administered for and on behalf of HCF by McKesson Asia-Pacific Pty Ltd.

What will the program provide?

This program works with your doctor’s treatment plan and will assist you with:

Understanding your doctor’s care plan
Understanding your condition
Addressing your heart health queries
Maintaining a healthy blood pressure
Understanding your cholesterol
Managing your weight
Living a healthy lifestyle
Quitting tobacco use if you smoke

The program components are:

Member Registration and Provider Contact Consent
Assessment Calls
24 x 7 Support, Triage and Health Advice
Educational Material
Goal-setting Action Plans
Telephone-based Coaching

Who is eligible to participate?

You will be eligible for participation if you:

Belong to an HCF hospital product held for at least 12 months
Have congestive heart failure or coronary artery disease
Are 30 years and over

What does HCF do with your personal information?

All information that you provide while participating in the HCF Healthy Heart Program will remain confidential and private. Your personal information will only be seen by our clinical consultants and will be used to deliver the HCF Healthy Heart Program to you. We may disclose your personal information, with your consent, to your health care providers (usually your GP) who will assist you in managing your health care program. The information you provide will also be used to administer and evaluate the program and will only be used by HCF for the purposes set out in HCF’s Privacy Policy.

HCF and McKesson Asia-Pacific Limited are committed to complying with the Privacy Act and the privacy policies of HCF and McKesson Asia-Pacific.
